The primary purpose of the Quality Coordinator position is to plan, organize, develop and direct the performance improvement activities, and safety of the organization to ensure that the highest degree of excellence in care is maintained at all times. The coordinator must also ensure that the day-to-day performance of key health care service delivery processes meet key performance requirements, including regulatory, payer, and Hospice requirements. Salary commensurate with experience and education.
Must possess, as a minimum, a Nursing Degree (RN) from an accredited college, university, or nursing school.
Must has, as a minimum, 2 years of experience in a hospital or health care setting
Prior Performance Improvement experience preferred.
Full-Time LPN (Home-Based)
The primary purpose of your job position is to implement and evaluate a patient’s care and patient/family needs through systematic assessment in order to promote comfort and health to patients and their families following the plan of care formulated by the Interdisciplinary Team and Staff RN.
Provides patients and families with skilled nursing care and other support services.
Teaches and counsels patient and family in nursing care and other related issues.
Responsible for reviewing patient chart before visiting patient and speaking directly to the Staff RN and Clinical Care Coordinator for patient update.
Must be available for on-call duties as assigned by the Clinical Care Coordinator.
Responsible for triaging phone calls from nurses, home health aides, patients and physicians.
Cross trains, understands, and complies with expectations for roles for whichhe/she is covering.
Works closely with on-call, admission, case manager and inpatient nurse and is flexible regarding shift changes and coverage.
Travel required with reimbursed mileage. Competitive benefits package and salary commensurate with experience.
Education and Experience:
A graduate of an accredited school of professional nursing who is currently licensed as a LPN in the State of West Virginia. Minimum of one (1) year of professional experience in the delivery of nursing services in Hospice, Home Health or Hospital, preferably two (2) years.
Full-Time Day Shift RN (Home-Based)
Registered Nurse candidates must work well in a team environment and be willing to provide hospice care in patient homes. Travel and on-call required. Competitive benefits package and reimbursed mileage. Salary commensurate with experience.
Registered Nurse (RN) Dayshift (Bowers Hospice House)
Hospice of Southern WV is currently seeking candidates for a full-time dayshift Registered Nurse (RN) in the Bowers Hospice House.

Be a part of an interdisciplinary team of skilled and caring people. Candidates must work well in a team environment and provide “a special kind of caring” to patients in the Bowers Hospice House during a day or night shift on an “as-needed” basis.
Part-Time Massage Therapist
Part-time massage therapist needed to join an interdisciplinary team of caring and compassionate hospice care providers. Must work well in a team environment and be available for up to 15 hours per week on a part-time basis to provide massage therapy within patients' homes. Successful candidates must posses a valid massage therapy license in the state of West Virginia. Travel is required and mileage is reimbursed.
