Office Nurse (LPN)

Requirements: 
Ability to triage patient calls and direct to appropriate staff
Work closely with outside agencies for securing durable medical equipment
Work closely with area pharmacies for obtaining medication needs for patients
Ability to take verbal orders from physicians and convey patient data
Assist Clinical Coordinator with office tasks
Multi-tasker
Excellent organizational qualities
Proficient with computers and electronic medical records system, preferably Consolo
Outstanding telephone skills

Preferred, but not required:
Experience managing a medical office 
Knowledge of hospice services

 

Community Liaison

The primary purpose of the position will be to assist in referral development through cultivating, maintaining and stregthening relationships with referral sources i.e. physicians, nursing homes, assisted living facilities, hospitals and home health agencies. The Community Liason is expected to educate the community at large on the hospice concept of care.
