Inpatient Clinical Care Coordinator

The primary purpose of the Inpatient Clinical Care Coordinator (ICCC) position is to plan, organize, develop and direct the overall clinical operation of the inpatient facility to ensure that the highest degree of quality care is maintained at all times. As part of an interdisciplinary team of skilled and caring people the Inpatient Clinical Care Coordinator (ICCC) must act in accordance with current applicable federal, state, and local standards, guidelines and regulations and as may be directed by the Chief Executive Officer and/or the Medical Director. The ICCC must also ensure that the day-to-day performance of key health care service delivery processes meet key performance requirements, including regulatory, payer, and Hospice requirements.


Registered Nurse (Home Based)

Be a part of an interdisciplinary team of skilled and caring people. Registered Nurse candidates must work well in a team environment and be willing to provide hospice care in patient homes. Travel and on-call required. Competitive benefits package and reimbursed mileage. Salary commensurate with experience.


Full Time “On Call” Registered Nurse (Home Based)

Be a part of an interdisciplinary team of skilled and caring people. Registered Nurse candidates must work well in a team environment and be willing to provide hospice care in patient homes. This positions primarily works weekends on an “on-call” basis. Travel to patients' homes is required. Competitive benefits package and reimbursed mileage. Pay commensurate with experience.


LPN (Home Based)

Be a part of an interdisciplinary team of skilled and caring people. Candidates for the Home-Based LPN position must have impeccable customer service skills to provides “a special kind of caring” to patients in the their own homes. Travel is required. Salary to commensurate with experience and mileage reimbursed.


PRN Nurse (Bowers Hospice House)

Be a part of an interdisciplinary team of skilled and caring people. PRN candidates must work well in a team environment and be willing to provide hospice care in Bowers Hospice House on an as-needed basis. Salary to commensurate with experience.
